Home Nurse (RN/LPN)

We are seeking a compassionate and reliable Home Nurse to provide high-quality nursing care to patients in their homes. The ideal candidate will be skilled, independent, and dedicated to improving patient well-being.

Responsibilities:

• Monitor patient health and record vital signs
• Administer medications and prescribed treatments
• Perform wound care and other clinical procedures
• Assist with daily living activities as needed
• Educate patients and families on care plans
• Report changes in patient condition promptly

Requirements:

• Active RN or LPN license
• CPR/BLS certification
• Previous nursing or home health experience preferred
• Strong communication and assessment skills
• Ability to work independently

1. Assist patients with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting.
2. Monitor and record vital signs including temperature, pulse, respiration, blood pressure, and oxygen saturation.
3. Support mobility by helping patients with ambulation, transfers, and positioning to prevent pressure sores.
4. Maintain patient hygiene and ensure a clean, safe, and infection-controlled environment.
5. Assist with feeding patients and monitor intake and output (I&O charting).
6. Observe and promptly report any changes in patient condition to the registered nurse or physician.
7. Provide basic wound care and assist in procedures under supervision of a licensed nurse.
8. Offer emotional support and communicate effectively with patients and their families.
9. Ensure proper documentation of care provided as per hospital or home care protocols.
10. Follow ethical standards, patient confidentiality, and safety guidelines at all times.
